• Please fill out one form per person.
• There is a 10 participant maximum for this event.
• A waitlist will be created if demand exceeds openings.
• Club equipment will be available for use.
• We regret that due to anticipated demand, we cannot offer this event to
• anyone who is not a ASC member or a direct relative
• (wife/mother/daughter/granddaughter/daughter-in-law) of a member.
